Enter the state, province, region or equivalent subdivision, if applicable.
Enter the date the business was legally incorporated.
Enter the approximate number of people employed by the business.
VAT number, EIN, or local equivalent corporate tax identifier.
Enter the business’s main website address. If the business does not have a website, explain why in the Business promotion description field below.
If the business does not have a website, explain how it promotes its products or services.
Explain how funds have been accumulated including by whoever the backer if that is how the business is funded.
State which bank and country the funds will be sent from.
Explain how the account will be used, including the currencies required and the countries payments will be sent to or received from.
Provide the signatory’s full name, residential address, email address, phone number, nationality, date of birth, tax ID, job title, director status and ownership percentage, if applicable.
Briefly describe what the business does, including its main products or services, customers and markets.

Beneficial Ownership

Provide details of anyone who owns 25% or more of the business or exercises significant control over it, such as a senior manager or executive. If there are more than three beneficial owners, include the additional details in the continution section.

Provide their full name, date of birth, nationality, email address, phone number, tax ID (if applicable), ownership percentage, residential address, director status or job title, relevant online profiles and any positive or negative press or professional highlights.

Provide the same information for the third beneficial owner, if applicable.
Explain the main reason the business will use the account, including the types of payments it expects to send or receive.

Enter the approximate total value of payments expected each month.
List all countries the business expects to receive payments from and state whether the payers will be individuals or businesses.

Confirm whether any cards will be issued to employees of the business.
